Acknowledgment of Risks

I understand that participation in volunteer activities related to food distribution, including but not limited to lifting, transporting, sorting, packing, and distributing food and supplies, may involve certain risks, including but not limited to:

​

• Slips, trips, and falls

• Lifting-related injuries

• Exposure to weather conditions

• Exposure to allergens or food-related substances

• Vehicle or transportation-related incidents

• Contact with members of the public

I voluntarily choose to participate in these activities and knowingly assume all risks associated with participation.

I agree to follow all safety rules, policies, procedures, and instructions provided by the Organization, including food safety, hygiene, confidentiality, conduct, and site-specific requirements. I understand that the Organization may remove me from volunteer activities at any time if it determines that my participation may create a safety risk, policy violation, or disruption.

Compliance with Rules and Instructions

I voluntarily assume all risks associated with my participation, whether arising from ordinary negligence or otherwise, to the fullest extent permitted by applicable law.

I further understand that volunteering in food distribution activities may involve inherent and other risks, known and unknown, including but not limited to lifting or moving heavy boxes or equipment; slips, trips, and falls; cuts, scrapes, or other minor injuries; vehicle-related risks during loading, unloading, or transportation; weather-related exposure for outdoor activities; exposure to allergens, dust, spoiled items, or communicable illnesses; interaction with members of the public; and other risks that may result in bodily injury, illness, emotional distress, death, or damage to personal property.

Release and Waiver of Liability

In consideration for being allowed to participate in the volunteer activities described above, I, on behalf of myself and my heirs, assigns, personal representatives, and next of kin, hereby release, waive, discharge, and hold harmless:

SERVE HAWAIʻI, including its board, employees, volunteers, partners, sponsors, affiliates, and agents (“Released Parties”),

from and against any and all claims, liabilities, demands, actions, causes of action, damages, costs, or expenses of any kind arising out of or related to any injury, illness, loss, or damage that may occur as a result of my participation in the volunteer activities, including transportation to and from the activities, except to the extent caused by gross negligence or willful misconduct of the Released Parties.

Indemnification

I agree to indemnify, defend, and hold harmless the Released Parties from and against any and all claims, damages, liabilities, losses, costs, or expenses (including reasonable attorneys’ fees) arising out of or related to:

1. My participation in the volunteer activities;

2. My negligent or intentional acts or omissions; or

3. Any breach of this Agreement by me.

Health and Medical Authorization

I certify that I am physically able to participate in these volunteer activities. I understand that the Organization does not provide medical insurance coverage for volunteers.

In the event of an emergency, I authorize the Organization to obtain emergency medical treatment for me (or the minor listed above) if necessary. I understand that I am responsible for any medical expenses incurred.

Photo and Video Release Authorization

I grant permission to SERVE HAWAIʻI, to photograph, videotape, and/or otherwise record me (or the minor listed above) during participation in volunteer activities.

I authorize the Organization to use these photographs, videos, recordings, or likenesses for lawful purposes including, but not limited to:

• Promotional materials

• Social media

• Website content

• Newsletters

• Annual reports

• Marketing and fundraising materials

I understand that no compensation will be provided for such use.
